Forms

  • Event Authorization Form
    This form must be submitted 14 days in advance for any event. To submit this form you must have participated in PGP Training. For those of you who participated in PGP Training but would like a refresher the PowerPoint can be found by clicking here.
  • Grade Release and Non-Hazing Form
    This form must be completed to add any new members or transfer members to your chapter roster and is due within 7 days of them joining your organization.
  • Student Liability & Release Waiver
    If you are coordinating or providing transportation for an event, this form must be completed, or if you are hosting an event with some liability involved.
  • Intermittent Domestic Travel Waiver
    Members must complete this waiver once per semester if they are traveling out of town with the chapter or to a chapter function.

On-Campus Room Reservations

  • Stern Student Center Room Requests
    This link will allow you to request meeting space at the Stern Student Center. You must complete the Facility Request Form and receive all correspondence, including confirmation of reservation, in order for the event to take place. Your chpater's on-campus advisor should be included to approve this request.
  • Student Classroom Reservations
    This link will allow you to request classroom space through the registrar's office. Only registered/sanctioned student organizations may use College facilities. Your chpater's on-campus advisor should be included to approve this request.
